Three Easy Ordering Steps

• Step 1: Contact your Prosthetics Department and provide Advanced Respiratory, Inc.’s VA contract number. 
• Step 2: The Prosthetics Department will send the purchase order and consult Advanced Respiratory, Inc.
  • CONTRACTED PRODUCTS PO MUST INCLUDE:
    • Purchase price 
    • Credit Card number with expiration date 
    • Include FSS Contract # V797D-50350 
  • NON-CONTRACTED PRODUCTS PO MUST INCLUDE:
    • Purchase or rental price (Synclara Cough System only) 
    • Credit card number with expiration date
  • CONSULT MUST INCLUDE: 
    • Patient’s date of birth 
    • Last 4 digits of social security number 
    • Ordering physician’s name 
    • Patient’s diagnosis
