MDS2 form Requests:

• MDS2 form is the Manufacturer Disclosure Statement for Medical Device Security
• MDS2 forms may be shared with XXXXXXXXX customers on request
• MDS2 forms are part of the DHF (Design History File) in SAP for the associated product
• You must look up the current version of MDS2 form by product using SAP T-Code CV04N in order to send to any customer
